Hey! It looks like you a missed a portion of the form.
This form of joining would require a description of your character's physical appearance (which you have, though you might want to add a few more sentences of you can think of any), as well as a detailed biography for your character. This would include its history and the major occurrences in their life up until coming to 'Souls.
